The Lead Care Manager oversees the provision of Enhanced Care Management Program services for assigned cases; effectively coordinates Member’s services across multiple systems of care; provides education and advocacy; and works collaboratively with the multidisciplinary treatment team in ensuring Member receives high quality and seamless care.
Responsible for credentialling all sites with various governing bodies, ensuring sites are compliant with contracts, accreditation standards, licensing requirements, regulations and Five Acres’ policies and procedures. Responsible for collecting and collating information regarding contractor (MCP or DMH) services and liaising between programs and that contractor.
Responsible for working with children, young adults, and families to provide skill building services and implement effective treatment techniques to achieve desired outcomes.
This position will be responsible for developing relationships and engaging with a variety of managed health care plans, community partners, and community based organizations (CBOs) from current contracted Five Acres service areas/locations to promote agency services and generate client referrals for Five Acres.
Responsible for providing a wide variety of mental health services to children, young adults, and families including individual therapy, family therapy, collateral services, care coordination, and monitoring/guiding the treatment and interventions delivered by paraprofessional team members.
Responsible for providing medication, treatment, support and rehabilitation services to all residents.
Responsible for facilitation of youth groups and adults to ensure quality outcomes, permanency of children placed in adoptive homes.
