This position will be responsible for developing relationships and engaging with a variety of managed health care plans, community partners, and community based organizations (CBOs) from current contracted Five Acres service areas/locations to promote agency services and generate client referrals for Five Acres.

Responsible for purchases, installs, maintenance, decommission and repairs of computer systems, accessories and peripherals, software and customer support for systems used by our organization. Conducts new user IT onboarding, monitors and coordinates staff changes according to workflow. Supports and maintains user account information, including role-based access and security rights on all information technology systems. Replenishes and keeps accurate inventory of technology supplies and assets. Runs basic diagnostics, isolates problems, implements solutions, and performs preventative maintenance tasks. Second-level responder for user support and collaborates with higher tiers of support within team roles and expertise for more complex diagnostics and troubleshooting.

• Responsible for developing, implementing, monitoring, and adapting behavioral services with clients and families.
• Assist in the implementation of individual treatment plans for children/youth focusing on vital areas such as therapeutic interaction, crisis intervention, education, and activities.
• Effectively partnering with families so that they feel empowered as a vital member of a team.
• Collaborating with co-located DCFS staff to complete intake process and orientations for children being admitted to Therapeutic Shelter Homes.

Responsible for the oversight of therapeutic behavioral services in accordance with treatment goals, clinical assessment, clinical case documentation, professional standards and licensing requirements.

The Lead Care Manager oversees the provision of Enhanced Care Management Program services for assigned cases; effectively coordinates Member’s services across multiple systems of care; provides education and advocacy; and works collaboratively with the multidisciplinary treatment team in ensuring Member receives high quality and seamless care.

Responsible for managing and supervising staff including:

• Support of Five Acres’ strategic goals, mission, & values; ensuring quality service, audit, and fiscal performance.
• Clinical supervision to ensure provision of clinical, case management, advocacy, and related services to assigned clients in accordance with treatment goals, licensing requirements and professional standards.
• Individual/group supervision to ensure development, implementation and coordination of program services in accordance with Agency goals and quality standards.
