Private Health Care Rehabilitation Specialist

Essential Duties 

  1. Provides therapeutic, behavioral skill-building services to children, young adults and families via individual rehabilitation services and collateral services in accordance with client treatment plans.
  2. Continually assesses, monitors, and adapts behavioral interventions indicated the in-client treatment plan to ensure the most appropriate method(s) of intervention and individualized client treatment.
  3. Submits timely documentation as related to the caseload for the program, maximizing billing according to caseload notes and accurately documents all work with client and/or family and/or relevant persons as measured by QA standards and peer review audit results.
  4. Maintain regular weekly communication/consultation with clinicians in order to stay updated on client treatment, needs, and care coordination opportunities.
  5. Participates in case supervision by coming prepared to discuss current caseload, documentation and productivity; accepts constructive feedback and direction; and puts feedback into therapeutic practice with clients.
  6. Establishes and maintains rapport and effective working relationships with clients, families, and community; works effectively within the PHC position and community at large.
  7. Attends/participates in monthly treatment planning meetings (Child and Family Team Meetings and team consultations when necessary) and attends/participates in client psychiatric appointments with PHC team, clients, and families.
  8. Maintains knowledge of program standards and policies as developed by Five Acres, COA, Community Care Licensing and other governing bodies; develops personal and professional knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; benchmarking state-of-the art practices; participating in in-service programs.
  9. Completes client risk assessments in accordance with PHC program trainings/protocols. Ensure immediate communication of client risks to PHC team, PHC supervisors, and emergency mental health providers in the community.
  10. Maintains confidentiality over all information regarding clients, families, and operations of the Agency (including media).
  11. Incorporates Agency philosophy and mission in all aspects of job performance.
  12. Maintains professional relationship with clients, staff, and other outside stake holders at all times.
  13. Exercises good judgment in the performance of duties and responsibilities.
  14. Established professional goals and pursues professional opportunities to grow

Qualifications 

  • High School Diploma plus two (2) years of mental health experience (paid or unpaid) required. Bachelor's degree in a mental health related field, preferred. Must be 18 years or older.
  • Mental health experience includes experience providing services to children and/or adults to improve or restore social, vocational, economic, housing and/or mental functioning
  • Must have a valid California Driver License, proof of automobile insurance and meet the Agency's safe driver guidelines.
  • Availability of a personal car with appropriate insurance which employee is willing to use for all Agency business, if needed; adhering to federal motor vehicle safety standards when transporting children.
  • Working knowledge of the family patterns, child-rearing practices, special needs, traditions and values of the ethnic and cultural groups served by the program.
  • Working knowledge of the methods involved in organizing recreation activities.
  • Effective communication skills both oral and written.
  • Ability to be flexible and work in a changing environment.
  • Computer skills sufficient to meet production demands of the job including Microsoft Office and WELLIGENT software for program notes.
  • Ability to work varied hours including evenings, nights and weekends.
  • Strong problem-solving skills.

Compensation Range: $22.50/hr - $30.98/hr
