Private Health Care Clinician - South Bay

Essential Duties 

  1. Provides direct clinical work and/or therapeutic services to children, young adults and families using a team and community-based approach in accordance with the client treatment plan.
  2. Complies with caseload standards for program, maximizing billing according to caseload notes.
  3. Submits timely documentation (progress notes, assessments, treatment plans, notes to chart, CANS/outcome measure completion) as related to caseload and accurately documents all work with client and/or family and/or relevant persons as measured by QA standards and peer review audit results.
  4. Continually assesses and advocates for the needs symptoms of children, young adults, and families (meets medical necessity) formulating psychosocial diagnosis, gathering history, and determining the most appropriate method (s) of providing treatment/utilizing an Evidence Based Practice
  5. Modifies treatment via completion and submission of outcome measures, utilizing practice guides, and by adjusting client treatment plans and diagnoses in accordance with client presenting symptomology.
  6. Participates in clinical/case supervision by coming prepared to discuss current caseload, documentation and productivity; accepts constructive feedback and direction; and puts feedback into clinical and/or therapeutic practice with clients.
  7. Regularly attends/participates in monthly treatment planning meetings (Child and Family Team Meetings and team consultations when necessary) and attends/participates in client psychiatric appointments with PHC team, clients, and families to ensure that the PHC team is providing services in accordance with the client treatment plan.
  8. Establishes and maintains rapport and effective working relationships with clients, young adults, families, and community; works effectively within the Agency and PHC healthcare partner.
  9. Maintains knowledge of program standards and policies as developed by Five Acres, COA, Community Care Licensing and other governing bodies; develops personal and professional knowledge by attending educational workshops; reviewing professional publications; establishing personal networks; benchmarking state-of-the art practices; participating in in-service programs.
  10. Completes client risk assessments in accordance with PHC program trainings/protocols. Ensure immediate communication of client risks to PHC team, PHC supervisors, and emergency mental health providers in the community.
  11. Maintains confidentiality over all information regarding clients, families, and operations of the Agency and PHC partner (including media).
  12. Incorporates Agency philosophy and mission in all aspects of job performance.
  13. Maintains professional relationship with clients, staff, and other outside stake holders at all times.
  14. Exercises good judgment in the performance of duties and responsibilities.

Qualifications 

To perform effectively in this position, the incumbent must have:

  • MA Degree; Registration with the California Board of Behavioral Sciences (BBS) as an AMFT or APCC.
  • Must maintain CA BBS registration as a condition of employment.
  • Previous employment experience with children, young adults, and families.
  • Prior experience working in a private healthcare field preferred.
  • Trained or certified in MAP/Wraparound Model preferred.
  • Must have a valid California Driver License, proof of automobile insurance and meet the Agency's safe driver guidelines.
  • Availability of a personal car with appropriate insurance which employee is willing to use for all Agency business, if needed, adhering to federal motor vehicle safety standards when transporting children.
  • Ability to work effectively within a multicultural team and agency.
  • Working knowledge of community resources that address the special needs of the client base.
  • Ability to attend Day 1 of the Pro Act de-escalating system training.
  • Maintain First Aid/CPR Certification.
  • Effective communication skills both oral and written; bi-lingual skills in Spanish/English preferred.
  • Ability to be flexible and work in a changing environment.
  • Computer skills sufficient to meet production demands of the job including Microsoft Office and WELLIGENT software for program notes.
  • Ability to work varied hours including evenings, nights and weekends.
  • Ability to model positive behaviors and Agency values.
  • Strong problem-solving skills

Compensation Range: $68,640/yr - $100,058/yr
