Case Manager

Essential Duties 

  1. Assists clinicians with case management responsibilities; meets with the clinician for assigned clients on a regular basis.
  2. Ensures that client documents are scanned, uploaded into Welligent and filed in client's chart, per the chart model provided by QA as assurance of uniformity.
  3. Create charts for new clients and breakdown charts for discharged clients per QA standards.
  4. Assist the program with follow-up and monitoring/tracking of chart compliance for, DCFS and DMH, using Welligent reports.
  5. Position may additionally be required to handle electronic health records (EHR) maintenance; including client records searches.
  6. Will be responsible for attending program staff meetings as needed.
  7. Conducts periodic internal audits of DCFS and DMH client case records with a focus on required information, signatures, timeliness and accuracy.
  8. Manages client case records at assigned service sites; reviews documentation for required information, signatures and deadline compliance as required by the DCFS and DMH funding source contracts; reports findings to Wraparound Administration.
  9. Maintains records of internal audits and supports the Wraparound Administration with necessary follow, assessing audit results for trends and making system recommendations.
  10. Assists with all adjunctive program reports are received by the treatment plan review date to ensure that communication regarding the child's/families' treatment is represented at that meeting; inputs assigned information and reports into the clients' records.
  11. Makes necessary contacts with treatment team members (i.e., attorney's CSW, family members, agency volunteers, CASA Worker, auxiliary treatment programs, school personnel, etc.) as assigned by the supervisor; properly documents all contacts in the child's Case Activity Note.
  12. Meets with the clinicians and Clinical Supervisor and other treatment team members to ensure that the quality and continuity of care among clinical and case management needs and services are being met.
  13. Assists QA DCFS Analysts with tracking and archiving all discharged charts for storage.
  14. Assists with supervisor to:
    1. Project a positive image of Five Acres to the community and inform referral sources of upcoming vacancies as measured by expansion of current referral sources, number of clients referred for service and feedback from referring agents.
    2. Respond to and screen all possible referrals for service recording how many were appropriate, how many were not and why not.
    3. Review referral information: monitor and record all associated information.
  15. Maintains knowledge of program standards and policies as developed by Five Acres and other governing bodies.
  16. Incorporates Agency philosophy and mission in all aspects of job description and follows established practices, policies and procedures to achieve compliance with Agency expectations and professional standards.
  17. Attends and participates in department staff, supervision and support meetings, in-service programs and Agency meetings as appropriate.
  18. Receives feedback with commitment to on-going refinement and professional growth.
  19. Maintains confidentiality regarding children, families and operations of the agency.
  20. Exercises good judgment in the performance of duties and responsibilities.
  21. Maintains professional relationship with clients at all times.

Qualifications 

  • To perform effectively in this position, the incumbent must have:
  • Bachelor's degree in a related field of study, preferred
  • (2) years full-time paid relevant experience required
  • Knowledge of Department of Mental Health (DMH) documentation
  • Effective communication skills both oral and written; bi-lingual skills in Spanish/English are preferred.
  • Computer skills sufficient to meet demands of the job including Microsoft office and Welligent software for program notes. Skills in maintaining detailed, accurate, and complete records.
  • Ability to be flexible and work in a changing environment, work varied hours including evenings, nights, and weekends, and effectively as part of a team.
  • Must have a valid California Driver License, proof of automobile insurance and meet the Agency's safe driver guidelines.
  • Sensitivity to the different cultures represented among the clients, families, and staff.
  • Basic understanding of age-appropriate behavior for children/adolescents.
  • Knowledge of emergency procedures of the agency.
  • Ability to model positive behaviors and agency values, strong leadership and problem-solving skills.
  • Ability to work effectively as part of a team
  • Strong problem-solving skills

Compensation Range: $18.03/hr - $25.24/hr
