AMERICAN SIGN LANGUAGE CASE MANAGER (Bilingual ASL Required)
(40-hours/week) position under the general direction of the Program Supervisor.
Responsible for working with the treatment team to ensure the provision of case
management services for severely disturbed children and adolescents in the Deaf
Services Community Based Specialty Program at Five Acres in accordance with
policies and the clients' treatment plans.
Position responsibilities include, but are not limited to:
regularly scheduled treatment planning meetings with clinicians and treatment
team members to determine the needs of each child for case management services;
coordinates case management services to ensure the needed services are
provided, and necessary documentation is completed.
and maintains relationships with school districts and community organizations;
refers clients to appropriate community services; coordinates contacts and
visits; escorts clients to referral appointments when applicable.
service plan updates; monitors service plan activities and follows-up with
referral agencies regarding client progress to ensure client outcomes.
necessary contacts with treatment team members (e.g., attorneys, CSW, family
members, agency volunteers, CASA Workers, auxiliary treatment programs, school
personnel, etc.) as assigned by the clinician; documents all contacts in the
data and assists the team in data collection to track service outcomes;
completes monthly reports to capture program services.
and conducts parent/youth workshops as needed by school and neighboring
SSTs, IEPs, psychiatric visits, and community events to act as a liaison on
behalf of the treatment team of Five Acres as appropriate.
in scheduling psychiatric consultation appointments; transports children to
appointments and other sites to facilitate meetings; provides feedback
regarding meetings to the clinician; completes all necessary documentation in
the child's file.
monthly with the clinician and supervisor to ensure that the quality and
continuity of care among clinical and case management needs and services are
being met per the Child's Needs and Service Plan.
all Mental Health services accurately, completely and in a timely manner;
maintains appropriate progress notes and other records; complies with productivity standards and
billable hours in accordance with regulations and Five Acres policy
Position requirements include, but are not limited to:
degree in social services, psychology or related mental health field and four years'
experience working in a mental health setting; or AA degree with six years'
experience working in a mental health setting.

work experience working with severely emotionally disturbed children and their
oral/written communication skills
skills sufficient to meet production demands of the job including Microsoft
Office and TIER/WELLIGENT software, and skill in maintaining detailed, accurate
and complete records
